With the application referred to in paragraph 1, the Member State concerned shall submit: a description of the boundaries of the proposed protected zone, including maps; the results of surveys showing that, during at least the three years preceding the application, the quarantine pest concerned was not present in the territory concerned; and evidence that the quarantine pest concerned fulfils the conditions set out in Article 3 with respect to the proposed protected zone.
