An application dossier as referred to in Article 24(7) shall include: the name and address of the applicant group; the single document referred to in point (c) of paragraph 1 of this Article; a declaration by the Member State that it considers that the application meets the requirements of this Regulation and the provisions adopted pursuant thereto; the publication reference of the product specification.
