Section 391A(1)

An application for an order under section 391(1) or (2) must— be made to the Secretary of State in such form and manner as the Secretary of State may require, be accompanied by such information as the Secretary of State may require, and be supplemented by such additional information as the Secretary of State may require at any time between receiving the application and determining it.
