Schedule 13, paragraph 8(5)

A notice required to be served by sub-paragraph (4) above shall— indicate that the mineral site in question is an active Phase II site; and indicate the date specified in the second list in relation to that site as the date by which an application is to be made to the mineral planning authority under paragraph 9 below; explain the consequences which will occur if such an application is not made by the date so specified; and explain the right to apply to have that date postponed, and indicate the date by which such an application must be made.
