Schedule 16, paragraph 47(2)

The statement must— be verified by a statement of truth in accordance with Civil Procedure Rules, be in the prescribed form, give particulars of the company’s property, debts and liabilities, give the names and addresses of the company’s creditors, specify the security held by each creditor, give the date on which each security was granted, and contain such other information as may be prescribed.
