Schedule 4, Part 3, paragraph 20

A notice under section A10(1)(a) or A11(1)(a) of the Insolvency Act 1986 must state— the company's address for service, and the court (and where applicable, the division or district registry of that court) or hearing centre in which the notice is to be filed.
