Schedule 4, Part 4, paragraph 69(1)

An application by the directors of a company for the extension of a moratorium under section A13 of the Insolvency Act 1986 must state— that it is made under that section, the length of the extension sought, identification details for the company to which the application relates, the company's address for service, and the court in which the application is made.
