Schedule 8, Part 3, paragraph 31

An application by the directors of a company for the extension of a moratorium under Article 13CD of the Insolvency (Northern Ireland) Order 1989 must state— that it is made under that Article, the length of the extension sought, identification details for the company to which the application relates, and the company's address for service. The application must be authenticated by or on behalf of the directors (see paragraph 49).
