Schedule 8, Part 3, paragraph 52

Where a provision of this Part of this Schedule requires a document to contain contact details of a monitor or other office-holder, the following information must be given— a postal address for the monitor or office-holder, and either an email address, or a telephone number, through which the monitor may be contacted.
