Section 74.16(3)

An application under paragraph (1) must be accompanied— by a certified copy of the judgment issued under Schedule 7 to the 1982 Act; and by a certificate, issued not more than six months before the date of the application, stating that the conditions set out in paragraph 3 of Schedule 7 are satisfied in relation to the judgment.
