Section 76.13

The controlled person must give notice of appeal by— filing it with the court; and serving a copy of the notice and any accompanying documents on the Secretary of State. The notice of appeal must— set out the grounds of the appeal; and state the name and address of- the controlled person, and any legal representative of that person. A notice of appeal may include an application for an order under rule 76.19 requiring anonymity. The notice of appeal must be filed with— a copy of the order that is the subject of the appeal; a copy of the Secretary of State’s decision on an application for the revocation of the control order, or for the modification of an obligation imposed by such an order.
