Section 77.9

The application must be made by filing a claim form pursuant to Part 8. The claimant must file with the claim form— a witness statement which deals with the conditions in section 55(1), (2) and (4) of the 1996 Act and which exhibits any relevant documents (which may include a copy of any record of court proceedings); and a copy of the certification under section 54(2) of the 1996 Act.
