Section 89.11(1)

An attachment of earnings order must contain such of the following information about the debtor as is known to the court— and that information will be the prescribed particulars for the purposes of section 6(3) of the 1971 Act. the debtor’s full name and address; the debtor’s place of work; and the nature of the debtor’s work and the debtor’s works number, if any,
