Schedule 26A, paragraph 6

A final notice must specify— the grounds for imposing the variable monetary penalty; the amount of the penalty; the method by which it can be paid; the period within which payment must be made; the grounds on which the person on whom the penalty is imposed may appeal; and the consequences of failing to comply with the requirements of the final notice.
