A person who— commits an offence. without reasonable excuse, refuses or fails to comply with the requirement in paragraph (1), (2) or (5) (as the case may be); or knowingly or recklessly, gives information under paragraph (1), (2) or (5) (as the case may be) which is false in a material particular,
