Section 39(4)
The information is— the contracting authority information, the title of the procurement, the unique identifier for— the procurement, and the public contract, a description of the three key performance indicators set in accordance with section 52(1) which the contracting authority regards, at the time the contract performance notice is published, as most material to performance of the contract obligations, a statement that the notice is being used to set out the contracting authority’s assessment of performance against key performance indicators selected in accordance with sub-paragraph (d), for each supplier whose performance has been assessed against the selected key performance indicators— the supplier’s name, the supplier’s contact postal address and email address, and the unique identifier for the supplier, the contracting authority’s assessment of performance against the selected indicators in accordance with the ratings set out in paragraph (5), and the time period to which the contracting authority’s assessment applies.
